Cristina Perez, RN, MSN, FNP-C, is board-certified by the American Association of Nurse Practitioners as a family nurse practitioner at Carelon Health in Upland, California. Ms. Perez completed her master’s degree at Walden University in Minnesota. Her primary area of expertise is adult and geriatric nephrology and advanced primary care in the chronic kidney disease, end-stage renal disease, and post-kidney transplant population. She is an active member of the American Nephrology Nurses Association and the California Association of Nurse Practitioners. “My goals are to offer care that goes beyond medical treatment and provide personalized care for positive outcomes,” she says.
